Connect Well West Kent
Connect Well West Kent is a Social Prescribing project managed and delivered by Involve Kent, in partnership with NHS West Kent Clinical Commissioning Group (CCG).
Social prescribing is a way to help people make positive changes in their lives and within their communities by linking them to voluntary and community groups and other non-clinical services.
Connect Well West Kent will have specialist staff at selected GP practices, supported by volunteers, to advise and support patients to get involved in community activities, volunteering, or accessing community services.
This will in turn allow you to:
- Feel more involved in the community
- Meet new people
- Make some changes to improve health and wellbeing
