New Patient Registration
We are currently accepting new patients who live within the practice catchment area.

Registration forms
If you wish to register at Warders Medical Centre please either complete our online GP registration form or contact the practice to request a registration pack.
You do not need proof of ID to register with us, but it would help if you have one or more of the following:
- passport
- birth certificate
- HC2 certificate
- rough sleepers' identity badge
- hostel or accommodation registration or mail forwarding letter
- proof of address
If you're homeless, you can give a temporary address, such as a friend's address, a day centre or the GP surgery address.
You may book an appointment with a GP as soon as you are registered, but we ask that you wait at least a week so that your previous medical records can be accessed by us.
Your Named GP
The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing other doctors as your named GP will not be available at all times.
We will make reasonable efforts to accommodate your personal preference for which doctor you wish to be assigned to.
