We have taken the decision not to accept any new patients, except new additions to households currently registered with us in our Practice area; this is to ensure that our current patients have access to appointments and that they are treated safely and continue to receive the highest possible quality of care.

If you are still unable to find a Practice locally, please contact the Primary Care Allocation Service on 01634 335095 or visit this webpage.

For patients who meet the criteria as above, we would ask them to please contact the practice to request a registration pack. Please ensure the pack is completed and returned to the practice with the relevant ID (photographic proof of identity, such as your passport or driving licence and proof of residency; this could be a recent (within three months) bank statement or utility bill) at least a week before making an appointment. This will enable the medical team to use the computer system when they see you, and potentially have access to the electronic notes from your previous surgery.

Your Named GP

The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing other doctors as your named GP will not be available at all times.

We will make reasonable efforts to accommodate your personal preference for which doctor you wish to be assigned to.
